Shifts in healthcare delivery in Ontario mean that Primary Health Care Nurse Practitioners will be able to contribute to health promotion and disease management for persons of all ages in rural and urban communities.
This innovative PHCNP is part of a provincial consortium of university partners and York’s program will prepare advanced practice nurses with expertise in primary health and with specialized knowledge in health promotion, empowerment, community partnerships, human science nursing, and interdisciplinary collaboration.
