Atkinson Students' Association - Mandatory Health & Dental Plan

The Atkinson Mandatory Health & Dental Plan

The Atkinson Students’ Association has listened to its members (you) who voted 85% in favor of a mandatory health and dental plan. This plan provides extended health and/or dental coverage for each individual Atkinson student enrolled in a course load of 6 credits or more on campus for Fall/Winter 2006/2007. The charges for the plans will appear on your Passport York student account statement.

Opting Out of the Plan

You can opt out of the plan only if you have existing comparable extended health insurance (not including UHIP or OHIP). To opt out of either the health and/or dental plans, you must fill out a waiver form and provide proof of coverage (i.e. copy of your benefits card or statement from your insurer) by the applicable deadline. All waivers and documentation must be received by the Atkinson Students Benefits Office by the deadline in order to be opted out of the plan.

In person (RM 104, Atkinson College)
By fax (416-650-8253) NOTE: you must keep your fax transmission as proof of faxing waiver/documentation prior to the deadline.
